States divergent in plans for health care ruling

CHICAGO (AP) ? As the nation awaits the Supreme Court ruling on President Barack Obama's health care overhaul, states across the country are considering how they will respond to the historic decision. Some Democratic-led states vow to push ahead with various provisions no matter what happens. In some Republican territories, elected officials insist they will try to hold off on implementing the law, even if the court upholds it. And most states are bound to miss key deadlines if the law or even pieces of it survive.

PUSHING AHEAD:

One contingent of states plans to stay the course regardless of the high court ruling.

Oregon's Legislature voted in 2009, a year before Obama's overhaul was approved, to create an online state health insurance exchange. If the court strikes down only the federal requirement that almost everyone obtain health insurance, the state's exchange director Rocky King says he still can build a profitable exchange to launch in 2014.

"We assume we can go forward," King said. "It may make a difference in terms of the number of people we can enroll, but we think the exchange has value with or without the mandate."

If the court strikes down the entire health care law, however, the exchange would lose more than $60 million in federal grants and thousands of potential customers. In that scenario, state officials would have to wait for cues from the Obama administration and for Congress to decide a path forward, King said.

California has been a model for the federal health care overhaul since the law's passage and has begun implementing parts of it already, including the beginnings of health care exchanges. The state also has banned insurers from refusing coverage for children with pre-existing illnesses and allowed young adults to stay on their parents' plans through age 26.

California passed its own state legislation to ensure those measures are preserved even if the federal law is thrown out. But that doesn't mean the state is unaffected by Thursday's ruling. As one of the law's biggest financial beneficiaries, it stands to gain as much as $15 billion annually in federal money for health programs.

Hawaii also has been moving full speed ahead and joined several states last year in filing a friend-of-the-court brief in support of the new law.

"Our goal is universal health coverage here," said Beth Giesting, who was hired as a coordinator to implement the overhaul's changes in Hawaii.

In Massachusetts, which laid the groundwork for the federal health care law with a sweeping 2006 state program, officials say they will continue efforts to expand coverage to nearly all residents no matter what. But like California, the state could lose hundreds of millions in assistance if the federal law is knocked down.

"Even in a worst-case scenario of the health care law being struck down entirely, Massachusetts is going to forge ahead," said Glen Shor, executive director of the Commonwealth Health Insurance Connector Authority.

DIGGING IN:

On the other side are Republican leaders who vow that no matter how the Supreme Court rules, they will not act on the overhaul before November, when they hope the GOP can win back the White House and the Senate and repeal the law.

Republican Wisconsin Gov. Scott Walker said this week that only in the event of a GOP defeat would Wisconsin "figure out some alternative within the state."

Robert Kraig, executive director of Citizen Action of Wisconsin, a health care advocacy group that supports the law and expanding health insurance coverage, said the governor is "on the far end of the spectrum in terms of trying to block the law and doing nothing to prepare for it."

But Walker is not alone.

Lawmakers in Idaho, where about 19 percent of residents are uninsured, were the first in the nation to pass a law in 2010 requiring the state to sue the federal government over the overhaul. Last year, they toyed with the idea of nullifying the health care law and have taken no steps toward implementation since. Now they're banking on the Supreme Court turning down the measure or a future repeal.

South Dakota Gov. Dennis Daugaard, Florida Gov. Rick Scott and South Carolina Gov. Nikki Haley have said they too will wait until November to before acting on any surviving parts of the law.

MOVING MONEY:

Millions of people already have received new benefits under the law. But those could disappear it is struck down, and many states are trying to figure out what to do if that happens.

Minnesota Gov. Mark Dayton, a big supporter of the federal overhaul, extended Medicaid coverage last year to 84,000 vulnerable adults. But Dayton has said little publicly about the implications of the Supreme Court's ruling for Minnesota. That leaves many unanswered questions about the fate of the people Dayton switched to the joint federal-state Medicaid program from less generous state-funded programs.

A ruling against the law also would threaten other planned expansions, such as in Washington state, which received $128 million to help set up its insurance exchange and an additional $200 million for its preexisting-condition insurance pool. Officials are uncertain whether that money could stay with the state or would have to be returned.

In Michigan, the law has brought widespread benefits: More than half a million seniors have received some free preventive health care services this year. And 1.8 million residents now receive preventative services with no co-pay. Another 57,000 young adults in Michigan are on their parents' health insurance plans.

Last week, U.S. Health and Human Services director Kathleen Sebelius visited Detroit to announce grants for six Michigan health centers to help expand access to care under the law. Officials there aren't sure what they'll do if that money disappears.

Funding for North Dakota's 15 community health centers may be jeopardized, and three other planned centers might not be built if the federal law is overturned.

SWEATING DEADLINES:

Most states have taken few ? if any ? steps toward creating the insurance exchanges that are a cornerstone of the health law. The online marketplaces would allow people and small businesses to comparison-shop for health insurance.

So if the court upholds the mandate to establish exchanges, many states will have to scramble to get their plans approved by January and the programs running a year later. As of March, only 13 states and Washington, D.C., had adopted plans for exchanges.

In Obama's home state of Illinois, Democratic Gov. Pat Quinn considered an executive order to establish an exchange, but state Rep. Frank Mautino said it's too late. The first year of the project will have to be a joint state-federal exchange, he said.

In Michigan, House Republicans have refused to let state officials start setting up the MI Health Marketplace before the Supreme Court ruling. Gov. Rick Snyder is one the few Republican leaders who has acknowledged considering a state-federal partnership to get an exchange in place.

"I'm just trying to be a pragmatist here," Snyder said Tuesday.

In Indiana, Republican Gov. Mitch Daniels ordered state agencies to investigate the cost and feasibility of an online health insurance marketplace in January 2011, but Daniels told the agencies to keep their work in the planning stages until the court ruled.

When you are in conducting a credit card debt negotiation, its important to know that you are negotiating for a position of strength. The Credit card companies don't want you to default. They make their money by receiving consistent payments from you. Payments that can go on for years and years.

You are the one in control. You have something that they want to get from you, your payment. If your polite, professional and firm, you will get what you want.

This is a serious situation. They do have the ability to instigate legal action. This is unlikely on smaller balances, but is always a possibility. I have seen balances as high as 15,000 dollars end up being charged off without legal action. I have also seen balances as low as $ 500 go to court after just a few calls. But even court action is a fairly slow process, and you have time to react and negotiate the debt before the court date.

First, you need to make sure that you communicate by mail. Don't give the collector a "no way out" situation by telling them to never contact you again. You have them backed them into a corner where they have little recourse but legal action. Credit card debt negotiation involves a level of compromise.

Instead, tell them they can contact you only by mail. And that they can never contact you at work. You will need to draft a letter to this effect and send it to them by registered mail, return receipt. In fact, all future communication with the collector will be by registered mail. And you always want to make sure you have a file of the delivery receipts and a copy of the letter available so that they can't dispute that it was mailed, and that they received it.

The problem with phone calls is that when you are involved in credit card debt negotiation, you need to have verification. If it wasn't written, it wasn't done because you have no way to prove it if you end up in court.

Begin your credit card debt negotiation at 20-25% of the original debt amount. The collection agency may have added on fees and charges that you should not include in the settlement amount. These fees just amount to the collection agency trying to make more money, and have nothing to do with the debt to be settled.

Since on the "secondary market" debt is traded at literally a few cents on the dollar, the collection agency is making plenty of money, even if they end up settling at 30 or 40% of the original debt.

Don't sound to eager to settle. Be calm and collected. Don't get angry, don't show hesitation. One thing I really want to add is NEVER let them know that you have some specific reason for settling the debt. Many people make an effort to settle before applying for a home loan for instance. This information is none of the collectors business. If they find out that the reason you want to settle is so that you can buy a house, you will never get a settlement.

So, in review.

1. Never talk to a collector on the phone. Always use registered mail with a return delivery receipt.

2. Start your negotiation at 20 - 25% of the original loan amount.

3. Don't include the collection agencies add on fees as a part of your credit card debt negotiation.

4. Be calm, educate yourself to the process so that you can speak from a position of knowledge.

Just be patient. And make a good deal. Soon, this will all be behind you. Your credit card debt negotiation means you can get on with your life.

Tropical Storm Dokuri weakened into a tropical depression today, but NASA infrared satellite imagery is still showing some areas of strong convection and thunderstorms in the storm and they're being pushed away from the center.

NASA's Aqua satellite passed over Tropical Storm Dokuri on June 27 on 0435 UTC (12:35 a.m. EDT). Infrared data from the Atmospheric Infrared Sounder (AIRS) instrument onboard indicated there were still some strong showers and thunderstorms within the system. Those cloud top temperatures were colder than -63 Fahrenheit (-52 Celsius), indicating strong uplift in the atmopshere, and thunderstorms within Dokuri that have the capability for heavy rainfall.

Infrared data shows a fully-exposed low level-circulation center. The strongest convection (rising air that creates thunderstorms that make up the system)are being pushed to the southwest due to strong northeasterly vertical wind shear.

Warnings are up in the Philippines as Doksuri (known there as Dindo) continues to move north of Luzon today. Public storm warning signal #1 is in effect in Abra, Kalinga, Isabela, Ilocos Sur, Mt. Province, Ifugao, La Union, Benguet, Pangasinan, Nueva Vizcaya and Quirino. Public storm warning signal #2 is also in effect for Cagayan, Calayan Group of Islands, Babuyan Group of Islands, Batanes Group of Islands, Apayao, Ilocos Norte.

On June 28, 2012 at 1500 UTC (11 a.m. EDT), Tropical Storm Doksuri had maximum sustained winds near 35 knots. It was located approximately 335 nautical miles north of Manila, Philippines, near 20.4 North and 120.7 East. It was moving to the west-northwestward at 17 knots. The strongest convection and thunderstorms were located over northern Luzon at this time, and are moving off the northwestern coast of the Philippines.

Doksuri has weakened as it passes to the north of Luzon. Doksuri is expected to continue tracking west and make landfall near Hong Kong in two days.

Study identifies causes for high rates of allergic reactions in children with food allergies

A team of researchers from Mount Sinai School of Medicine and four other institutions have found that young children with documented or likely allergies to milk and/or eggs, whose families were instructed on how to avoid these and other foods, still experienced allergic reactions at a rate of almost once per year. Of severe cases, less than a third received epinephrine, a medication used to counter anaphylaxis, a life-threatening allergic condition.

The findings are from an ongoing Consortium of Food Allergy Research (CoFAR) study that has been following more than 500 children with food allergies since infancy. The results of the three-year study appear online in the June 25 issue of Pediatrics.

Nearly 72 percent of the participants experienced a reaction, with 1,171 allergic reactions in total. Allergic reactions were attributed to such factors as a lack of close supervision, misreading ingredient labels, cross-contamination, or errors in food preparation. Participating families had been given written and verbal food avoidance instruction, and written prescriptions for self-injectable epinephrine, beforehand.

"This study reinforces the importance of educating parents and other caregivers of children with food allergy about avoiding allergenic foods and using epinephrine to treat severe food-allergic reactions," said Scott Sicherer, MD, Professor of Pediatrics and Chief of the Division of Allergy and Immunology at Mount Sinai School of Medicine. "We must work harder to thoroughly educate parents about the details of avoidance and when and how to correctly use epinephrine to manage this life-threatening condition."

Approximately 11 percent of the children experienced anaphylaxis, which can include symptoms such as swelling in the throat, asthma, sudden drop in blood pressure, dizziness or fainting. The children's parents or caregivers administered epinephrine in only 30 percent of the cases of children having severe reactions to food. Investigators found children did not receive epinephrine because either the drug was not available, or parents and caregivers were too afraid to administer the drug, or they did not recognize the reaction as severe and waited to see more symptoms.

"We found a significant number of young children received allergenic foods from caregivers other than their parents," said Hugh Sampson, MD, Dean for Translational Biomedical Sciences, Professor of Pediatrics, and Director of the Jaffe Food Allergy Institute at Mount Sinai School of Medicine. "This underscores the need to educate everyone who is responsible for the child, including grandparents, older siblings and teachers."

Dr. Sampson is the lead investigator for COFAR, which is also performing numerous food allergy treatment trials.

The study is taking place at The Mount Sinai Medical Center as well as research hospitals in Baltimore, MD; Denver, CO; Durham, NC; and Little Rock, Ark., with support from The National Institutes of Allergy and Infectious Diseases (NIAID) of the National Institutes of Health. NIAID conducts and supports research?at NIH, throughout the United States, and worldwide?to study the causes of infectious and immune-mediated diseases, and to develop better means of preventing, diagnosing and treating these illnesses.

Racine community hosted fundraiser for Travon Gray?s family

RACINE ? A taco dinner fundraiser was held Friday, June 22nd to help the family of Travon Gray with funeral costs. 16-year-old Travon Gray died Sunday, June 17th after drowning in Quarry Lake.

Gray was at Quarry Lake Park on Sunday, and jumped off of the pier with some friends. Divers were called to the scene to search for Gray. He was later pronounced dead at a hospital.

Ebony Jeans came up with the idea of holding a taco dinner fundraiser to help the Gray family with funeral expenses. The group she founded, the Divine Cake Social Club, signed on immediately.

?Basically, we are a women?s group and we are here to support and uplift each other. But also give back to our community,? said Jeans.

The Gray tragedy hit close to home for Jeans. She says Gray and her son were best friends.

?It?s a friend that he had that came over every day, you know, he ate dinner with us. He hung out. I joked around with him all the time and now he?s not going to be around, so,? said Jeans.

Jeans hopes to raise enough money for the meals and donations to cover half of Gray?s funeral expenses.

Obama key figure in Senate race featuring ex-govs

(AP) ? The dominant figure in a Virginia Senate race that could determine the direction of Congress next year is President Barack Obama even though both candidates for the seat are former governors well-known to voters.

Republican George Allen, trying to redeem his political career and win back the Senate seat he lost six years ago in a chaotic, slur-stained campaign against Democrat Jim Webb, has a simple strategy: If Obama falls, Democrat Tim Kaine falls with him.

Kaine, like Allen a former governor, was Obama's hand-picked chairman of the Democratic National Committee for two years. Their fates in November are linked in a battleground state that both parties consider critical to winning the White House as well as controlling the Senate.

That makes Virginia the perfect spending target for the super political action committees on each party's side, a chance to bag two opponents at once using the same dollars. As go Obama and Republican Mitt Romney in the president race, so go Kane and Allen.

Allen denies Kaine even the Virginia courtesy of addressing those who've held their office as "governor," calling him instead "Chairman Kaine," a derisive reference to his two years as head of the Democratic Party, one of them during his final year as governor. An Allen billboard in rural areas is even blunter: "Obama's senator, not Virginia's."

"Look, for a quarter of his term as governor, he took on the most partisan job in our country," Allen said in a telephone interview. "He was President Obama's hand-picked chairman, and he's been an advocate for all the significant agenda items of the Obama administration."

Kaine has never tried to disown Obama, though they disagree on several issues. He chafes at assertions by Allen and others that the two are joined at the hip.

"It is not surprising that George is doing that," Kaine said in a telephone interview. "The last thing George wants to do is talk about the fact that he was a U.S. senator and he has a record as a U.S. senator and what that record was."

Obama's drop in popularity over his health care law, stimulus package and clean-air initiatives despised by Virginia's coal industry have made the link to the president tougher for Kaine.

"Everyone has a fantasy," said Virginia-based Republican strategist Christopher J. LaCivita. "Tim Kaine's fantasy is not to be associated with Barack Obama."

Tying Kaine to Obama is a good Republican strategy unless Romney flops in Virginia, said Steve Jarding of Harvard University's John F. Kennedy School of Government. Polling by Quinnipiac University in early June showed Obama with a 5 percentage point lead over Romney in Virginia and Kaine and Allen deadlocked.

"If Obama loses big, there's probably no way Kaine can win, and vice versa ? if Obama wins big, there's probably no way George Allen can win," said Jarding, a Democratic consultant.

Kaine's ? and Obama's ? hopes hinge largely on big turnouts by women and minorities. One-fifth of Virginia's electorate is black, and among that group, 9 out of 10 voted for Obama in 2008. Democrats hold an advantage among female voters, according to a Quinnipiac University poll of Virginia in June. Obama led Romney by 16 percentage points among women, and Kaine led Allen by 6 points.

Among the 60 or so in attendance at the exclusive event held at Glenwild Golf Club, was Congressman Paul Ryan, R-Wisconsin, in town to support presidential candidate Mitt Romney and his weekend fundraising efforts, and Love at her Friday night event.

Ryan brought along some generous donors and the event hauled in more than the $65,000 that was expected to help boost Love's campaign in Utah's 4th Congressional District. The evening's VIP reception came at a $2,500 asking price, with attendance to Ryan's speech at $1,000 a head.

Ryan, chairman of the House Budget Committee, spoke of the need to get the country headed back in the right direction, and to re-establish the American Dream as an opportunity and not something the government hands out, according to one attendee.

"We want to extend the universal ideals of this country, as an opportunity society with a safety net, a society of upward mobility, a society where we promote equal opportunity," Ryan told the Deseret News following the event. "I think Mia Love is a perfect ambassador of those ideals."

Ryan said Love offers a unique approach to the GOP and he said Washington supports her.

Love sent her husband, Jason, to speak to the group in her stead as the fire burned in Utah County. He hand-carried and delivered her words: "The people of Saratoga Springs come first. They need me tonight and I am here with them."

Ryan declined to comment on whether he is considering a run on the vice presidential ticket, but he is one of several possible picks participating in a Utah retreat for Romney's biggest donors and fundraisers that continues through Sunday. To be invited, supporters must have given at least $50,000 or raised a minimum of $250,000.

Other GOP heavy-hitters gathered this weekend at Deer Valley include the 2008 presidential nominee, Arizona Sen. John McCain; strategist Karl Rove, former U.S. Secretary of State Condoleezza Rice and former Florida Gov. Jeb Bush.

Federal officials say that two health insurers owe consumers in the state more than $1.1 million.

About 32,600 people in Idaho will get rebates on their 2011 health insurance premiums this summer, the Centers for Medicare and Medicaid Services said Thursday.

That?s an average $70 for each of the 16,000 affected families, officials said.

According to the agency, 1,083 Idahoans with individual policies will get $144,303, averaging $323 per family. About 31,500 people with small-group plans will get $980,615, or $63 per family.

Despite the agency?s announcement, the payments are uncertain. That?s because the law that requires the rebates ? the 2010 federal health care reform law ? might not survive. The Supreme Court is expected to rule within a week on the law?s constitutionality, which was challenged by Idaho and other states.

Only if your insurer failed to hit a ?medical loss ratio? threshold, meaning it devoted too few of your premium dollars to medical care. Generally, insurers that spent more than 20 percent of premium revenue on administration, salaries, sales and advertising must give rebates by Aug. 1.

Two companies will owe money to people in Idaho, according to the agency. It didn?t identify the companies, but Regence BlueShield of Idaho told the Idaho Statesman that it is one.

Regence spokeswoman Georganne Benjamin said the insurer will provide rebates totaling $980,000 to its members in Idaho small-group insurance plans. That is less than 1 percent of the members? premiums and about 0.03 percent of premiums companywide, Benjamin said.

?As a nonprofit insurer, it is our goal to price premiums as close to break-even as possible when determining expected claims and administrative costs,? Benjamin said. ?This has been our practice before the (new law) and will continue to be our practice regardless of any federal and/or state reforms.?

The other Idaho insurer sells insurance for individuals, the agency said.

About two-thirds of people nationwide were insured by companies that met the 20 percent standard, according to federal officials. Blue Cross of Idaho was one, said spokeswoman Karen Early. People covered by Blue Cross of Idaho shouldn?t expect rebates because the company spent ?more on its members? health care than required by law? last year, she said.

How will I get my rebate?

One of four ways:

? A check in the mail.

? A lump-sum reimbursement to the credit or debit card account used to pay premiums.

? A reduction in future premiums.

? A pass-through benefit from an employer, if the employer provided the health insurance policy.

50-gigapixel camera is straight out of science fiction

While photographers marvel at the 20 or 30 megapixels captured by the latest cameras, researchers at Duke laugh quietly and return to their lab to continue work on a system that could capture 50 gigapixels at the touch of a button.

For comparison, if your average point-and-shoot picture was the size of a word on this page, a 50-gigapixel shot would be around the size of your entire screen.

Gigapixels aren't new in and of themselves, of course: Enormous photos of the Milky Way or Obama's?inauguration are both effective demonstrations of the power of huge images. But generally these giant pictures are created by one camera that takes hundreds (or thousands) of shots, which are then?fitted together after the fact. Not a problem if you're taking pictures of stars, but no good?if you wanted to capture a flock of birds taking off.

The team at Duke, led by David Brady at Duke's Pratt School of Engineering, decided to take things a different direction: instead of taking lots of pictures in sequence with one camera, why not take lots of pictures at once with a bunch of cameras?

The result is the AWARE-2 camera system, a machine that looks like something out of science fiction. It bristles with wires and ports, and houses?a lens array that could be the eye of an enormous robotic insect.

The system actually shares many characteristics with insects' compound eyes: A single image enters through a central lens, but is then refracted outward in 98 different directions to 98 different image sensors, each of which receives a different piece of the picture. The pieces are then assembled by a central processor.

Images produced by the system are enormous. 50 gigapixels is the maximum theoretical size, but even a fraction of that?that is far larger than any picture most people have ever taken. Look at the shot below; on this enormous landscape, the tiny white dots on the sky are in fact birds, captured in enough detail that you can zoom in and check out the details of their wings. That's with a prototype device using only half the microcameras and the first generation of lenses designed for the system.?Brady explains that right now, the size and quality of images is limited by the quality of the main objective lens, but they have already built a superior lens for 10-gigapixel images.

50 gigapixels is the upper limit because it is the precision?limit of lens-manufacturing technology, and also because beyond that you start running into the resolution limit of the atmosphere, i.e. what can be discerned through all those pesky air molecules between the camera and the subject. So it's no surprise that they're considering imaging at that size for astronomical rather than terrestrial photography.

The applications of enormous images captured more or less instantly are easy to imagine, both in everyday life, academic research, and for the military, which is funding the project through DARPA.

The paper describing AWARE-2 was published in the journal Nature, and the authors have put together a separate?site with more information and lots of?pictures for the curious.
